

{"id":1364,"date":"2020-03-28T04:16:01","date_gmt":"2020-03-28T09:16:01","guid":{"rendered":"https:\/\/staging.advancedrenaleducation.com\/wparep\/?post_type=article&#038;p=1364"},"modified":"2025-05-09T17:04:11","modified_gmt":"2025-05-09T22:04:11","slug":"gastrointestinal-complications","status":"publish","type":"article","link":"https:\/\/staging.advancedrenaleducation.com\/wparep\/article\/gastrointestinal-complications\/","title":{"rendered":"Gastrointestinal Complications"},"content":{"rendered":"<p>Delayed gastric emptying is relatively common among both diabetic and non-diabetic patients.\u00a0 Delayed emptying of both solids and liquids has been detected in symptomatic and asymptomatic peritoneal dialysis (PD) patients(1,2).\u00a0 A mechanical or neurogenic mechanism triggered by the presence of intra-abdominal fluid is thought to retard gastric emptying(3\u20135), although this has not been shown in all studies (6,7).\u00a0 Prokinetic medications are the mainstay of therapy for symptomatic cases.<\/p>\n<p>Pancreatitis is an infrequent but serious event often associated with peritoneal dialysis(8\u201310).\u00a0 Although there are several potential risk factors for pancreatitis\u2014including the presence of peritoneal dialysis fluid (PDF) in the lesser sac, irritants in PDF, peritonitis, hypertriglyceridemia and hypercalcemia\u2014there is controversy regarding a causative relationship between PD and pancreatitis(11). The clinical manifestations are abdominal pain with or without cloudy fluid, culture negative peritonitis, markedly elevated serum amylase (&gt; 3x normal levels), increased amylase in the dialysis effluent (&gt; 100 U\/L) and dialysate leukocytosis with sterile culture.\u00a0 Traditional therapy is indicated without significant changes in the PD prescription.<\/p>\n<p>Several other gastrointestinal complications have been described including ischemic colitis(12)and necrotizing enteritis, related to hypoperfusion of the bowel following hypotension; gastrointestinal bleeding from dilated submucosal vessels in the bowel(13); pneumoperitoneum with or without pain resulting from free air infused with the dialysis fluid(14); and hepatic subcapsular steatosis among patients receiving intraperitoneal insulin(15).<\/p>\n<div class=\"vcex-spacing\" style=\"height:30px\"><\/div><div class=\"vcex-module vcex-divider vcex-divider-solid\" style=\"width:100%;margin-top:20px;margin-bottom:20px;border-top-width:1px;border-color:#dddddd;\"><\/div>\n<h4>References<\/h4>\n<ol>\n<li>Guz G, Bali M, Poyraz NY, Bagdatoglu O, Ye\u011fin ZA, Do\u011fan I, Atasever T, Sert S, Sindel S.
