{"id":1227,"date":"2020-03-26T04:03:14","date_gmt":"2020-03-26T04:03:14","guid":{"rendered":"https:\/\/staging.advancedrenaleducation.com\/wparep\/?post_type=article&#038;p=1227"},"modified":"2025-05-09T15:57:04","modified_gmt":"2025-05-09T20:57:04","slug":"air-embolism","status":"publish","type":"article","link":"https:\/\/staging.advancedrenaleducation.com\/wparep\/article\/air-embolism\/","title":{"rendered":"Air Embolism"},"content":{"rendered":"<p>Advancements in technology and equipment have significantly decreased the chances of air embolism during hemodialysis.\u00a0 Air embolism during renal dialysis is extremely rare due to the safeguards built into the equipment and procedures that are currently used.\u00a0 In a study by Tennankore et. Al the number of treatments to have one event was less than 1 in 20,000 (1).\u00a0 Although air embolism is rare, it is important to understand how it develops in order to properly treat it and prevent it.<\/p>\n<h4>Etiology<\/h4>\n<ul>\n<li>Can be venous or less commonly, arterial<\/li>\n<li>Three vulnerable areas of air entry in dialysis patients:\n<ul>\n<li>Between patient and blood pump\n<ul>\n<li>Due to high negative pressure and leaks in the circuit in this segment<\/li>\n<\/ul>\n<\/li>\n<li>Air in the dialysate fluid (uncommon, mostly gets trapped in venous chamber)<\/li>\n<li>During central venous catheter insertion or removal<\/li>\n<\/ul>\n<\/li>\n<li>Upright body position and hypovolemia both reduce venous pressure and are significant contributing factors<\/li>\n<\/ul>\n<h4>Patient Presentation<\/h4>\n<ul>\n<li>Coughing, shortness of breath<\/li>\n<li>Chest pain or pressure<\/li>\n<li>Tachycardia<\/li>\n<li>Distended neck veins<\/li>\n<li>Cyanosis\/gray color<\/li>\n<li>Slight paralysis on one side of the body (cerebral)<\/li>\n<li>Confusion, convulsions, coma<\/li>\n<li>Possible cardiac\/respiratory arrest<\/li>\n<\/ul>\n<h4>Treatment<\/h4>\n<ul>\n<li>Prevent further air entry by clamping and disconnecting the circuit (2\u20134)<\/li>\n<li>Flat supine position may be better over traditionally advocated left lateral (Duran\u2019s position) and Trendelenburg position (2\u20134)<\/li>\n<li>Oxygen with FiO<sub>2<\/sub> 100% (4)<\/li>\n<li>Hyperbaric oxygen (prevents cerebral edema) (4,5)<\/li>\n<li>Use of Luer-lock syringes for blood draw from catheters (6)<\/li>\n<\/ul>\n<h4>Prevention<\/h4>\n<ul>\n<li>Test machine prior to use to ensure that the air detector alarm system is working effectively<\/li>\n<li>Catheter insertion or removal should be in a head low position (insertion site 5 cm below right atrium).\u00a0 Patient can assist by holding their breath or doing a Valsalva maneuver that will increase central venous pressure (4)<\/li>\n<\/ul>\n<div class=\"vcex-spacing\" style=\"height:30px\"><\/div><div class=\"vcex-module vcex-divider vcex-divider-solid\" style=\"width:100%;margin-top:20px;margin-bottom:20px;border-top-width:1px;border-color:#dddddd;\"><\/div>\n<h4>References<\/h4>\n<ol>\n<li>Tennankore KK, D\u2019Gama C, Faratro R, Fung S, Wong E, Chan CT.
